申请者或父母其中一人出生于霹雳州。
申请者或父母其中一人不出生于霹雳州但已在霹雳州居住 10 年或以上。
申请者或父母其中一人出生于霹雳州，现在已在其他州属居住，但曾在霹雳州居住 10 年或以上。
现居住在霹雳州的申请者将获优先权。
申请者家庭收入不可超过 RM5,000。
申请者须在大学第一年的第一学期申请。
申请者大学须获得来自 JPA 和 MQA 认证的大学。
凡涉及 Program Pertukaran Pelajar / Mobiliti Pelajar / Program Berkembar / Latihan Industri 的申请者没有申请资格。
助学金会根据申请者的学位分发。

申请程序

申请者可前往官方网站填写申请表格。
申请者须把相关文件通过邮寄 / 传真 / UTC 邮政局 / 亲自呈交到柜台的方式发送到 Yayasan Perak 办公室。（最迟从开始申请当天开始算起1个星期）
任何出现不完整或不齐全的文件等问题，即不被接受。
申请者可以在申请一个月后上网查询申请状态。

Eligibility of Application

Dermasiswa Yayasan Perak (Dalam Negara)

Only open to applicants who applied in the first semester of the first year of the university and enrolled in a full-time university course.
Applicants or one of his parents was born in Perak.
Applicants or one of his parents was not born in Perak but has resided in Perak for 10 years or more.
Applicants or one of his parents was born in Perak and now live in another state, but has resided in Perak for 10 years or more.
Applicants must be full-time students.
Applicants who have started university courses at least 3 semesters or more than 1.5 years of study time left. (limited to long semesters)
Applicant’s family income must not exceed RM5,000.
Dermasiswa will be distributed according to the applicant's degree.
Applicants who are currently living in Perak will be given priority.

Dermasiswa Yayasan Perak (Luar Negara)

Applicants or one of his parents was born in Perak.
Applicants or one of his parents was not born in Perak but has resided in Perak for 10 years or more.
Applicants or one of his parents was born in Perak and now live in another state, but has resided in Perak for 10 years or more.
Applicants must be full-time students.
Applicants who have started university courses at least 3 semesters or more than 1.5 years of study time left. (limited to long semesters)
Applicant’s family income must not exceed RM5,000.
Applicant's university must be from a university recognised by JPA and MQA.
Applicants involving Program Pertukaran Pelajar / Mobiliti Pelajar / Program Berkembar / Latihan Industri are not eligible to apply.

Documents Required for Application

Applicants’ copy of university offer letter.
Applicants’ and parents’/ guardians’ copy of NRIC.
Applicants’ copy of bank account statement.
Applicants’ families’ copy of salary slip. If self-employed, please attach proof of salary. If retired, please attach a proof of retirement.
Applicants’ parents’ / guardian’s copy of death or divorce certificate. (if relevant)

Procedure for Application

Applicants may visit the official website to fill in the application form.
Applicants are required to send the relavant documents to Yayasan Perak office by mail / fax / UTC post office / submit at the counter. (at the latest one week from the day of application)
Any incomplete documents will not be accepted.
Documents that have been submitted will not be returned.
Applicants can check the status of application via online one month after applying.
