表格须附上州议员、国会议员、州议员助手、村长、村委会主席、社区委员会主席或住宅区委员会主席的盖章证明

Eligibility of Application
Applicants must be residents of Negeri Sembilan.
Applicants should apply for this scholarship in the first academic year.
Applicants’ total family income not more than RM4,000
Applicants must fill in the PK-PSUKNS-31 form
If the applicant is a child of FELDA family, the applicant must fill in Borang Pengesahan Kelayakan Bantuan Pendidikan FELDA.
The form must be accompanied by the seal of the state member, member of parliament, assistant of state member, village head, village committee chair, community committee chair, or housing committee chair.

Applicant’s copy of Identity card
Applicant’s parent’s or guardian’s copy of Identity card
Applicant’s copy of parent or guardian salary slip
Applicant’s copy of university offer letter and university tuition fee receipt
Applicant’s copy of primary/ secondary school leaving certificate

The form can be obtained from the Office of the State Assembly or visit the official website of the Negeri Sembilan State Government to download the form.
